SSI Risk Management Instructions

• Go to www.usclubsoccer.org
• Login
  • If you are not already a member, then register as a "member"
• Click on Staff Management
• On the left hand side, click on "Background Check Instructions"
• In the middle of the text of instructions, click on the link (in blue type) that says "Staff Background Check Application"
• Register Type: select either Coach or Assistant Coach
• Click on "Go"
• Type of Membership: click on "League Team"
• On the right:
  • click on "League"
  • scroll down and select "Suffolk Soccer Interleague - Youth Super Rec (NY) #126"
• Click on "Club" and scroll to "Amityville"
• Complete all of the information, answer all questions accurately and click "Submit"
